Thought Recording Log

This Thought Recording Log should be used to record and assess your thoughts and feelings between therapy sessions as discussed with your therapist. These records are for review during sessions and are used to enhance therapeutic outcomes. You can submit as many as you need to between sessions.

Your individual client reference number for on-line anonymity
What emotions did you feel at the time? How intense were they on a 0-10 scale? What did you notice in your body? Where did you feel it?
What went through your mind? What disturbed you? What did those thoughts or images mean to you, or say about you or the situation? What were you responding to? What 'button' was that pressing for you? What was the worst thing about that, or that could happen?
What are the facts? What facts do you have that the unhelpful thoughts are totally true?
What facts do you have that the unhelpful thoughts are NOT true? Is it fact or opinion? What have others said about this?
What would someone else say about this? What is the bigger picture? Is there another way of looking at this? What advice would you give to someone else? Is your reaction proportionate to the actual event? Is this really as important as it seems?
What are you feeling now on a scale of 0-10? What could you do differently? What would be more effective? What would be most helpful for you or the situation? What will the consequences be?
